Among its preferred provider organization (PPO) individual insurance plans are BluePreferred, BluePreferred Saver (HAS Compatible) and BluePreferred Basic (High Deductible). BluePreferred is a plan with co-pays for Preferred office visits and five deductible choices. BluePreferred Saver is a high-deductible plan that meets federal requirements for use with a health savings account (HSA). You can have three deductible options with this plan. BluePreferred Basic is a low-premium high-deductible plan with three deductible choices. In the category of indemnity plans, you can have BlueClassic with six deductible options or BlueClassic Saver (HAS Eligible), which is a high-deductible indemnity plan that meets federal requirements for use with an HSA. A $5,000 deductible is available. BlueSelect is a health maintenance organization (HMO) plan with two plan options. BlueSelect Plan 2 requires co-pays for most covered services. BlueSelect Plan 3 is a lower premium HMO; deductible and coinsurance apply to inpatient facility charges. All Blue Cross Blue Shield of Arizona individual health plans include visits to doctor office, hospital benefits, prescription medications (no formulary), chiropractic benefits, routine physical exam and preventative screenings and well-child care and routine immunizations.
