plan in New Jersey, which provides health insurance coverage to
more than 3.2 million people spread over all of North, Central,
and South Jersey. Since its inception in 1932, the company has been
offering health care products and services keeping pace with the
changing needs of its customers. Horizon Blue Cross Blue Shield
of New Jersey offers its customers managed care, Medicare, and traditional
indemnity health plans for individuals, families, and groups. The
company has won a number of awards, including one for having the
top ranking in the New Jersey HMO Performance Report in 2004 and
the 2004 Brand Excellence Award from the Blue Cross Blue Shield
Association. Horizon Blue Cross and Blue Shield of New Jersey also

Horizon Blue Cross Blue Shield of New Jersey offers a wide range
of plans that include:
Horizon Basic Plan A/50: This plan is deductible-based plan with
which members can seek medical care from any provider. Even out-of-network
providers may be consulted, albeit at a higher out-of-pocket cost.
Under Horizon Basic Plan A/50, there is a yearly cap on coinsurance
should you or a member of your family need sudden or unexpected
serious medical care.
Horizon Traditional Plans B, C, and D: These traditional plans allow
members to choose from deductibles of $500 to $2,500 with co-payments
that can change according to the member's choice of deductible.
In-network providers are covered for a lower out-of-pocket rate
than non-network providers. Under Horizon Traditional Plan B, your
insurer is due to pay 60% of the costs after the deductible has
been met. Horizon Traditional Plan C will pay 70% of the costs after
a customer’s deductible has been met. Horizon
Traditional Plan D will pay 80% of your medical costs after your
deductible has been met.
Horizon MSA Companion/High Deductible Plans C and D: These plans
are a combination of traditional health plans with Medical Savings
Accounts. With these plans, members are free to choose from a range
of deductibles.
Horizon HMO: It is a health maintenance organization (HMO) plan.
With Horizon HMO, members choose a primary care physician (PCP)
from among over 10,000 doctors across New Jersey.
Traditional Indemnity Plans: It is a plan that reimburses members
for care after their deductibles have been met.
Horizon Blue Cross and Blue Shield of New Jersey offers under its
Basic and Essential EPO and EPO Plus Plans the freedom to choose
any physician or specialist in the network without needing authorization
from a primary physician. You are allowed to visit a physician who
is not a member of the network in the case of an emergency.
Horizon Blue Cross and Blue Shield of New Jersey was the first insurance
company in the state to offer Medicare supplemental insurance after
Medicare was first signed into law. It now covers over 160,000 Medicare
beneficiaries throughout all 21 counties of New Jersey, more than
any other company in the state.
