  • This plan belongs to the PPO type.
  • Patient will get $35 to pay for a General Physician’s visit and $50 for a Specialist.
  • Patient will get $500 for the prescribed drugs of Levels 2, 3 and 4.
  • Coverage for people outside the network is also available.
  • Coverage for people outside country is available only for the emergency cases.
  • Facilities for Periodic Health Exam.
  • 20% coinsurance for baby care.
  • 20% deductible coinsurance for hospitalizing a patient.
  • 20% deductible coinsurance for surgery of a patient.
  • Special coverage for mental health and chiropractic treatments.

Scheme 2: Portrait Share 80/1000 Plus RX & Unlimited Doctor Visit Copay

  • Available in: Iowa, Kansas, Louisiana, Michigan, Missouri, Nebraska, Ohio, Oklahoma, South Carolina, Texas, and Wisconsin.
  • This plan belongs to the PPO type.
  • Patient will get $35 to pay for a General Physician’s visit and $50 for a Specialist.
  • Patient will get $500 for the prescribed drugs of Levels 2, 3 and 4.
  • Coverage for people outside the network is also available.
  • Coverage for people outside country is available only for the emergency cases.
  • Facilities for Periodic Health Exam.
  • 20% coinsurance for baby care up to the age of 6 years.
  • Lab and X-Ray tests are free up to $200 of the expenditure.
  • 20% deductible coinsurance for hospitalizing a patient.
  • 20% deductible coinsurance for surgery of a patient.
  • Special coverage for mental health and chiropractic treatments.

Scheme 3: Monogram Total/7500 Plus RX

  • Available only in Florida.
  • This plan belongs to the PPO type.
  • No charge for the visits of General Physicians or Specialists.
  • Patient will get $1000 for the prescribed drugs of Levels 2, 3 and 4.
  • Coverage for people outside the network is also available.
  • Coverage for people outside country is available only for the emergency cases.
  • No charge for Periodic Health Exam.
  • Child Health Supervision Services for the children up to 17 years.
  • No charge for Lab and X-Ray tests.
  • No charge for hospitalization and surgery of a patient.
  • Special coverage for mental health and chiropractic treatments.

HumanaOne Plan with Office Visit Copay, $0 RX deductible

  • Available only in Nevada.
  • This plan belongs to the PPO type.
  • Copay for General Physician and Specialist are $25 and $40 respectively (coinsurance 0%).
  • Coverage for people outside the network is also available.
  • Coverage for people outside country is available only for the emergency cases.
  • $300 maximum for Periodic Health Exam every year (coinsurance 20%).
  • $300 maximum for baby care every year (coinsurance 20%).
  • Lab and X-Ray tests are free up to an expenditure of $100; coinsurance deduction of 20% begins after that.
  • 20% deductible coinsurance for hospitalizing and surgery of a patient.
  • Deductible coinsurances for mental health and chiropractic treatments are 50% and 20% respectively.
