  • Thrill Seeker (also called 5000): This plan allows the customers to access four doctor visits every year. This plan requires the customer to pay a copayment of $20 along with a deductible of $5,000. The premium rates are different at different states of the country. Those who reside at Connecticut needs to pay around $105.27 and $161.04 every month as the average premium. Nevadans are required to pay a monthly premium of $113 and those who stay at New Hampshire have to pay between $130.09 and $134.65 each month on an average.
  • Part-Time Daredevil (also called 3000): Policyholders needs to pay a deductible of $3,000 plus a copayment of $30. This plan offers four doctor visits annually to the customers. Connecticut residents need to pay around $116.56 and $179.46 every month as the average premium. Nevadans have to pay a monthly premium of $133 whereas at New Hampshire, the average monthly premiums range between $142.46 and $147.54.
  • Calculated Risk-Taker (also called 1500): In this plan, the number of doctor visits offered to the policyholders is unlimited. This plan can be owned by paying a deductible of $1,500 and $40 as copayment. Connecticut residents need to pay a monthly premium between $130.75 and $202.63 on an average. Nevadans have to pay $153 as the monthly premium while those at New Hampshire have to pay around $159.24 and $165.01 as the average monthly premium.
